Care and funding for adults with complex health needs
NHS Continuing Healthcare is a package of care funded solely by the NHS. It supports adults who have been assessed as having a primary health need due to disability, accident, or illness.
The NHS arranges and funds care based on assessed health and related social care needs. This means eligibility depends on the level and complexity of needs, rather than a specific diagnosis or where care takes place.
Care may be provided in a person’s home, care home, nursing home, or other appropriate setting.
Who is eligible for NHS Continuing Healthcare
You, or someone you care for, may be eligible if you have significant and ongoing health needs that require care to be arranged or overseen by the NHS.
Healthcare professionals carry out a formal assessment using the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care.
During the assessment, professionals will:
- review clinical evidence
- assess care needs and associated risks
- consider professional recommendations
- include the views of the individual and their family or carers
This process ensures decisions reflect individual care needs.
What support NHS Continuing Healthcare can provide
If the NHS confirms eligibility, it will arrange and fund a care package to meet assessed needs. Depending on individual circumstances, support may include:
- care at home
- care in a care home or nursing home
- specialist nursing care
- equipment and adaptations
- support to maintain independence
The care team will review care packages regularly to ensure they continue to meet changing needs.
